Monkey business going on at the Mint. Some employee may be creating fantasy pieces to make a quick buck. Proof coins, like this 1976-S nickel, are supposed to be thoroughly inspected before being inserted into a proof set holder.
Ive always believed that the rarest most ridiculous error coins were intentionally created. Not like being a government worker some how makes you honest and ethical.
